It is mandatory that all applicants for an Airman Medical Certificate complete their form online prior to their appointment for their physical examination. The form may be completed at the following link: https://medxpress.faa.gov/. The first step in the process is to establish an account with the FAA (if you do not already have one) by choosing a password. Once that is accomplished, log in and complete the form. When setting up the account and filling out the form, please be absolutely certain that key information (such as last name, first name, middle name, date of birth, SSN etc.) is entered correctly. After you have filled in all of the required information and answered all of the questions, click on the button that says “Check for validation errors.” If any errors are found, make the necessary corrections. Then check again for validation errors, and if none are found, enter your password at the bottom of the page and click on the “Submit” button. A page will then appear with your “confirmation number.” You must enter this number in the scheduling module of Dr. Miriani’s website and bring it with you when you come in for your examination. If you have not provided your confirmation number to Dr. Miriani and do not have it with you at the time of your examination, the examination will not be performed and you will not get a medical certificate!! On the same page that contains your confirmation number, you will see a button labeled “Exam Summary.” If you click on this button, it will bring up a page that looks just like the old paper form complete with all of your information. Although it is not mandatory that you bring a copy of this page with you to your examination, you may want to print this page if you wish to have a copy for your own records. Dr. Miriani will not be able to make a copy for you at the time of your examination. As an alternative to printing, you can save the page as a PDF file for future reference, since much of the information on the page will not be retained for viewing at the time of future applications.
The form may be completed up to 60 days prior to the examination. The date that will be on the medical certificate is the date of the examination, not the date that you filled out the form, so the date of the latter is inconsequential as long as it is within 60 days of the examination.
Please note that the correct answer to question 17b regarding near vision contact lenses is “NO” for 99.9% of all pilots.
